AMA Checklist for Transition to E/M Documentation and Coding Changes in 2021
January 6, 2020 – An online checklist and associated resources are now available through the American Medical Association (AMA) to assist physician practices in preparation for the upcoming changes to Evaluation and Management (E/M) services in 2021, including telemedicine. The AMA has been working with CMS to simplify E/M requirements with the goal to make them clinically relevant while reducing excessive documentation. The key principles of the E/M service changes include:
- Eliminating history and physical exam as elements for code selection;
- Allowing providers to select whether their documentation is based on medical decision-making (MDM) or total time; and
- Modifying MDM criteria to focus on tasks affecting the management of the patient’s condition rather than simply adding tasks up.
